This is a reminder to all Food Pantry programs that the new 2016-2017 JFS 04221 TEFAP form goes into effect tomorrow, July 1. All existing client-signed TEFAP forms must be removed from circulation and archived for a period of five years. Any blank forms dated 2015 or before must be destroyed to prevent accidnetal usage.
PLEASE REMEMBER THAT NO MODIFICATIONS, CHANGES OR ATTACHMENTS MAY BE MADE TO THE NEW 2016-2017 FORM.
